What is Xanthelasma?

Xanthelasmas are yellow-orange plaques or spots of 1mm to 3cm in diameter that they appear mainly on the upper and lower eyelids. Xanthelasmas arise from intracellular and intradermal lipid deposition and are the clinical manifestations of hyperlipidemia (high cholesterol levels). Histologically, they are characterized by the deposition of lipids in macrophages (foam cells).

Despite the fact that cholesterol over 200 is common in the western world, only a small percentage of patients develop xanthelasmas. However, their presence must necessarily lead to a thorough investigation of the patient, so that it can be accurately diagnosed and treated in a timely manner.

According to most studies, the lipids found in xanthelasmas are the same as those circulating in the blood. To transport plasma lipids, complex macromolecular compounds are formed that are called lipoproteins. Protein metabolism involves both exogenous (dietary) and endogenous pathways.

Heredity has not been fully elucidated, but xanthelasmas in the same family are common.

The most likely mechanism for the formation of xanthelasmas is lipoproteins that circulate in the blood to penetrate the wall of the capillaries of the skin and to be phagocytosed by macrophages, which thus end up becoming foam cells.

The most common ages are from 40 to 60 years old while at younger or older ages they are rare.

They occur in both sexes, but women have up to twice the incidence.

Taking cholesterol-lowering drugs does not mean that it will affect xanthelasmas.

They are benign lesions that never progress to cancer and are an aesthetic rather than a functional problem
But they do not go away on their own, they do not shrink, on the contrary they can grow and become more. For this reason, we recommend their immediate removal with Laser and specifically with the innovative CO2 Laser.

TREATMENT

In Athens South Dermatology Clinic, the diagnosis and treatment of xanthelasmas is always performed only by the specialized Dermatologist / Laser Dermatologist Surgeon Dr. Nektarios Logaras.

The procedure is non-invasive, completely painless, bloodless, without complications (ecchymosis, oedemas), without cessation of daily activities, eliminating the possibility of affecting vision (Diplopia).

By carefully examining and studying each xanthelasma, with the help of the technologically advanced imaging system Foto Finder, the doctor will allow to control the extent of each xanthelasma.

Then, with the use of local anaesthesia (never general anaesthesia is applied), in the microsurgery department of the well-organized medical center and always taking all the appropriate safety measures, he will proceed to the removal of the lesions, leaving the healthy skin intact; selectively removing, with the help of medical the entire contents of the xanthelasma, cauterizing – evaporating at the same time the adjacent involved tissues, in order to minimize their relapse.

After the end of the removal, what is left is essentially a dry uniform skin surface, which is whiter than the adjacent, but with the use of a regenerating cream for a few days, the skin regains it uniformity.

Precautions to prevent the formation and recurrence of xanthelasmas:

-Balanced diet (saturated fats must be avoided)
-Physical exercise
-Cholesterol regulation
-Stop smoking
-Restriction to a minimum of alcohol consumption
-Maintenance of low body weight
